Enrollment Procedures

We want to make the enrollment process as easy as possible for you! Here is a list of items you will need to have with you in order to complete the enrollment process:

1. Proof of Residency
This includes: a California Drivers License or State I.D. reflecting your Culver City address, 2 of your most recent utility bills (must be from 2 separate utility companies) or, if you don't pay for your utilities, 3 pieces of computer generated business mail. This is typically the type of mail you receive in a window envelope, for example: bank statements, credit card statements, insurance premium due notices, mail from any government agency, and any other monthly bills.

You will also need your Title of Property or most recent Property Tax bill if you are a homeowner, and your Lease Agreement if you are a renter. If the lease is not in your name, then the person who's name is on the lease must accompany you when you come to enroll and bring all of the proof of residency documents mentioned above in their name. They will be establishing their residency in Culver City and you will be establishing your residency in their home, so you will both need to have the applicable documents in order to complete registration.

2. If your child is an incoming kindergartner or first grader, you will need to have your child's doctor complete a CHDP form. Registration cannot be completed without it, and we will be happy to discuss the requirements on the form with you when you pick up your enrollment package. If your child will be attending grades 2-5, the CHDP form is not required, but you WILL need your child's complete immunization records when you enroll.

3. You will need to have your child's birth certificate or passport to complete registration. Please bring the original documents--we do not accept photocopies.
